QM sent me a incident report that was among other things needing to be scanned in to records. Report states that on 8/8/11 Veteran A recived prescription of Veteran B. Report states that the medication was mailed from Providence. Veterans had the same last name. Update: 08/25/11:One Veteran will be sent a letter of notification.
