Issue:

On 08/06/14, the Privacy Officer, Nashville Campus, was notified of the following incident: On 08/04/14, Veteran A presented to the Outpatient Pharmacy, Nashville Campus, to pick up his medications at the window. Instead, he was given medication belonging to Veteran B. Later, Veteran B presented to the window to retrieve his medication and the error was discovered. Veteran Bs medication was redone and dispensed to him. Veteran A had already left the hospital. Pharmacy staff attempted to reach Veteran A by phone but were unsuccessful. Pharmacy staff left several messages. That afternoon, Veteran A returned the call and was informed of the error. Veteran A had not taken any of the incorrect medicine and indicated would destroy the medication. Veteran Bs personally identifiable information (PII) that was compromised includes his full name, address, date of birth, provider name, and medication name.

Outcome:

08/07/14: The Incident Resolution Service Team determined that Veteran B will receive a letter offering credit protection services, since hsi full name and date of birth were compromised.
