Issue:
A report with the names, last four numbers of the SSN, year of birth ,and usernames of 42 individuals participating in the MyHealtheVet program was left on a printer from 2/2/2012 to 2/7/2012. A patient/Veteran working in the Compensated Work Therapy program found the documents and took them to an employee in the area. That employee forwarded the documents to the Privacy Officer Update: 02/07/12:Forty-two Individuals will be sent letters offering credit protection services due to their full name and date of birth being exposed.
Outcome:
Employee responsible reminded to be more cautious when choosing printer.
