Issue:

A person from the general public called our pharmacy to inform them that they'd received a prescription that wasn't theirs and was contained a label from our facility along with their medical from a civilian facility. The package was sent from our mail out pharmacy (CMOP) and we can only assume it was tampered with or damaged at the USPS sorting facility and the Veteran's prescription was placed in the same package as this civilian's prescription from Kaiser. Or if the CMOP services both VA and Kaiser, then this could have caused the mis-mailing. We're still investigating. Also a package was sent to the civilian's home to recover the Veteran's prescription. Update: 08/23/12:One Veteran will be sent a letter of notification.

Outcome:

No violation or corrective action required by our facility. The contracted mail order pharmacy has initiated an investigation into the event with the Tucson Post Office.
