Issue:

User sent email with personnel action out unencrypted. One of the recipients was chosen incorrectly from the global address list and the mail was sent instead to an individual working with the Navy Department on detachment to the VA at an outside facility. The mail was recalled and all other recipients were within the VISN 16 encrypted connections. Update: 03/30/12:Since the one recipient was uninteded, the employee named on the personnel action will be offered credit protection services. Their full SSN was disclosed.

Outcome:

Individual has been directed to complete PA refresher training.
