Issue:

A Provider, Charlotte Ave CBOC, received an appointment letter in the mail which he believed to be an error. The Provider is an Employee, not a Veteran, and receives all his care outside of the VA. As a result of having received the letter, Veteran is concerned other employees may have accessed his record inappropriately. The Provider requested a copy of a Sensitive Patient Access Report (SPAR) from December 16, 2012 to July 24, 2013. Per his written request, the Privacy Officer, Nashville Campus, provided the SPAR. Upon review of the SPAR, the Provider stated all eight (8) employees listed on the SPAR should not have accessed his record. The Provider requested PO investigate. The PO is entering this ticket as a complaint. If the PO is able to substantiate the accessions were inappropriate, the ticket will be escalated to an incident.

Outcome:

08/09/13: The employee will be sent a notification letter.
