Issue:

Veteran A came to his clinic appointment and brought a white plastic mailing envelope with his name on it with label stating VAMC Lincoln, NE 68510-2451. Inside this envelope were refill medication paper for accu-check plus a test strips for Veteran B as well as a second page listing prescriptions requested and dates when they would be mailed to him. This package was mailed 4-26-12. Veteran A kept it until he had his clinic appointment today. He reported he looked at it, knew he wasnt using insulin test strips so thought he would just bring it with him at his next clinic apointment. Update: 09/07/12:Veteran B will be sent a notification letter.

Outcome:

Staff was educated on the importance of disclosing information to the appropriate Veteran.
