Veteran A received records in the mail which belonged to Veteran B. The compact disc was labeled with Veteran As name but contained the records of Veteran B. Veteran Bs personal identifiable information includes: name, social security number, DOD, medical diagnoses, lab results, and medications. Veteran A returned the compact disc to the Supervisor, ROI.
Outcome:
01/27/14: Veteran B will be sent a letter offering credit protection services.
