Issue:

The Release of Information (ROI) clerk processed a request for a Veteran, but handed this Veteran the records of a second Veteran that the ROI clerk had just recently completed another request on. The first Veteran noticed that he had the wrong records approximately 15-20 minutes after ROI request was complete. The second veteran was contacted and found to have received his correct records. ROI clerk stated that she must have made an extra copy of second veteran's records, handed the first copy to him, and handed the extra copy to the next veteran, who subsequently returned to the ROI department and reported the incident. Update: 02/07/13:One Veteran will be sent a letter offering credit protection services.

Outcome:

ROI employees were counseled on proper procedures to double-check all documentation before providing it to patients, especially after obtaining information from printers/copiers.
