Today, Veteran A reported he received a patient notification in the mail letter which belongs to Veteran B. Veteran Bs letter was enclosed in the envelope with Veteran As information. Veteran A returned Veteran Bs document to the Clerk, Physical Medicine & Rehab, when he checked in for his appointment. Veteran Bs personal identifiable information includes full name, address, and providers name.
Outcome:
10/06/14: The Incident Resolution Service Team has determined that Veteran B will be sent a general notification letter.
