Issue:

On 08/08/12, the Privacy Officer (PO) of the Nashville Campus, was contacted by the Secretary, Chief, Business Office of the Murfreesboro Campus, who stated a Volunteer was currently in her office and wanted to report a privacy complaint. The Privacy Officer, Murfreesboro Campus, is out today. The Volunteer, who is a Clarksville Disabled American Veteran van driver, reported the following incident.Veteran A is a rider on the Clarksville van. Veteran A requested copies of his medical records from Release of Information (ROI), Murfreesboro Campus. Veteran A received his medical records in the mail but the packet also included a large volume of documents which belong to Veteran B. Veteran A gave the packet of information to the Volunteer to return to the Privacy Officer. Veteran Bs personal identifiable information include full name, full social security number, progress notes, lab results, and list of medications. The Secretary, Chief, Business Office (Murfreesboro Campus) will hold the medical records until the Privacy Officer is able to retrieve the documents. Update: 08/08/12:Veteran B will receive a letter offering credit protection services.

Outcome:

8/15/12 - PO found incident was due to a human error. The ROI clerk did not check the records for accuracy before mailing the documents. This is the clerk's first violation. Verbal counseling given by supervisor. CM letter mailed.
