Issue:

A controlled substance was lost in a Consolidated Mail Outpatient Pharmacy (CMOP) mismailing. A new temporary address was entered into VistA on that date by Pharmacy personnel; however, per the patient, it was the incorrect address. Pharmacy Management was notified by the patient that the medications had not been received/delivered.

Outcome:

04/06/15: The Incident Resolution Service Team has determined that the patient will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
