Issue:
Based on interview and record review, the facility failed to ensure the PHI (protected health information) of Patient A remained confidential. This failure resulted in the unauthorized disclosure of Patient A's PHI to an unintended recipient.Findings:An interview was conducted with the facility's Privacy Officer (PO) on March 25, 2014, at 8:50 a.m. The PO stated on March 18, 2014, a fax number was entered incorrectly resulting in Patient A's laboratory result being sent to the wrong individual.A review of Patient A's PHI which was faxed to the unintended recipient was conducted. The PHI reflected Patient A's name, medical record numbers, account numbers and results of a laboratory test.A review of the facility policy, "Faxing Protected Health Information (Last Reviewed/Revised 03/21/2013)," indicated, "All Protected Health information sent or received by (the facility) shall be in a manner that protects against unauthorized disclosure of such records to third parties..."
