Issue:

A holter monitor was packaged and sent via Contract Lab Courier in a locked box from a Community Based Outpatient Clinic (CBOC) to the Main Hospital. The lock box was delivered to the Lab but the holter monitor was never received by the Nursing staff responsible for the device. The lock box is not accessible by the Contract Courier so it is assumed to be lost within the facility. Update: 08/02/12:The device stored the full name, full SSN, Date of Birth and EKG readings of one Veteran. The one Veteran will be sent a letter offering credit protection services.

Outcome:

A credit monitoring letter has been mailed to the Veteran. A new process is being developed to track and secure the holter monitors. Since this ticket was entered, the holter has been found at the facility. The Veteran was contacted and informed that his sensitive information was never at risk.
