Issue:

Laboratory specimen with lab slip was lost during transport from patient care unit to laboratory. Update: 02/14/11:The lab slip included the Veteran's full name, full social security number, and date of birth. The Veteran will be sent an offer for credit protection services.NOTE: There were a total of 29 Mis-Handling incidents this reporting period. Because of repetition the other 28 are not included in this report, but is included in the "Mis-Handling Incidents" count at the end of this report. In all incidents veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

After investigation it could not be determine how the lab work was lost. An email is sent to all staff involved to safeguard the transporting of lab work within and on facility groundsThis is closed.
