Issue:

Pharmacy Service staff reported that Veteran A contacted the Pharmacy Service staff on 07/18/12 and advised that he did not receive his medication. The medication was shipped by UPS on 07/11/12. The medication was last scanned into UPS on 07/12/12 in Lancaster, SC as "out for delivery." The package included the medication and the instruction sheet with Veteran Bs full name and address and information on the medication and instruction sheet. Pharmacy Service staff will shipped out a replacement medication on 07/19/12. Update: 07/19/12:Although not the fault of the VA, one Veteran will be sent a letter of notification.

Outcome:

The VA Police, and IG have been notified.
