Issue:
On 03/08/11, VBA Reno (Las Vegas Office) requested a follow up on two active duty medical records but staff in the Compensation & Pension Office sent the records back to VBA Las Vegas Office via UPS on 01/27/11. VBA states they did not received the records.UPS tracking confirms the records were picked up on 01/27/11. No other tracking information is available on their website, including delivery information.Las Vegas reported the lost package to UPS on 03/08/11 and initiated a tracer request. The PII includes the complete military active duty medical record. Update: 03/10/11:The Privacy Officer is working with UPS. They have eight days to investigate the complaint of the lost package, beginning when the ticket was initiated on 03/08/11. Therefore the VA has to wait until UPS completes their investigation. 03/17/11:Documents were not found by UPS, no fault of the VA but since the Veterans full SSN was lost, two veterans will receive a letter offering credit protection services.
Outcome:
UPS conducted an investigation to no avail, package containing the medical records were not located and UPS closed tracking ticket. Credit monitoring letters mailed 3/17/11.
