Based on staff interview and record review, the facility failed to maintain the privacy and confidentiality for one person (Patient A) when a staff member accidentally gave Patient A's discharge instructions to another patient.Findings:An interview was conducted on October 5, 2012 with the facility's Privacy Officer (PO). The PO stated that on September 17, 2012 she was notified of a breach incident. The PO stated that on September 17, 2012 a staff member accidentally gave another patient six pages of Patient A's discharge instructions. The PO stated that the breach was identified when the other patient called back stating that they had someone else's discharge instructions attached with theirs.A review on October 5, 2012 of the personal health information (PHI) disclosed in error included: Patient nameChief complaintMedical record numberDiagnosisThe PO further stated that Patient A was notified of the breach occurrence.
