Issue:
A travel clerk was faxing the list of patients who would need transportation the following day which is provided by a contracted service. After faxing the list, the clerk realized they had sent the list to the wrong number. The clerk immediately called the number and was not able to reach anyone. The information was faxed to an appliance repair and it was after the companies office hours. First thing the following morning, the clerk called the number again and reached someone at the appliance repair company who informed the clerk that when they arrived in the morning and saw the fax they recognized it was not intended for them and the fax was shredded. Update: 04/26/13:Fifty-eight (58) Patients will be sent a general notification letter.
Outcome:
The partial SSN has been removed from the list and training conducted with the staff to verify the fax number before hitting send. Attached a sample copy of the signed notification letter.
