Issue:
Today 10-9-12, the Supervisor, Business Office, Nashville Campus, received a call from an ambulance driver of a contract ambulance company.On Sunday, October 7, 2012, while leaving TVHS, Nashville Campus, the ambulance crew inadvertently left a clipboard on the top of the ambulance. The clipboard fell off and a trip ticket was lost. The trip ticket was found by a bystander who immediately returned it to the Administrative Officer of the Day, Nashville Campus. The trip ticket contained a Veterans full name and last four SSN. Update: 10/09/12:The Veteran will be sent a notification letter due to PHI being exposed.
Outcome:
10/17/12 - TVHS was not responsible for the misplaced trip ticket. HIPAA notification letter mailed.
