Issue:

Protected health information (PHI) and prescription(s) belonging to Veteran A were misdirected to Veteran B. It is noted the Veterans have the same first and last names with different middle initials. Also, the Veterans demographic information indicates the same address for both Veterans to include same NOK and Emergency contact information, however, Veterans have different SSNs and DOBs. The package of information was mailed to the Health Resource Center Privacy Officer in Topeka, Kansas by Veteran B. The Privacy Officer at the HRC notified this Privacy Officer on this date. The package of information will be sent UPS overnight to this Privacy Officer for further assessment. Action to conduct a fact-finding has been requested to Medical Administration Service Business Office to determine the source and correct the demographic information for both Veterans; and, Pharmacy Service to determine the type of medications misdirected to Veteran B and which facility was responsible for the misdirect medication. Additionally, to determine if Veteran A's medication must be refilled stat.

Outcome:

11/14/13: Veteran A will be sent a notification letter.
