Issue:

Patient A received documents belonging to Patient B. Both veterans have the same last name and their orders were packed on the same day about one hour apart. The orders were packed by two different technicians located nine stations apart. This distance would limit the possibility of mixing the documents. It was noted on the day of processing these orders that a computer error occurred . The information technology staff explained that the error, occurring at the precise time, could have resulted in an unnoticed printing error. The packer of Veteran B's order did not alert the supervisor regarding lack of documents printing and there was no evidence showing reprinting of documents. Packers are not required to review the documents printed before placing them into the package. Therefore; this order was completed correctly. Veteran A was instructed to destroy the documents belonging to Veteran B. Action has been taken at Dallas Consolidated Mail Outpatient Pharmacy (CMOP) to prevent this error from reoccurring. Update: 05/24/11:Patient B will be sent a notification letter, due to full name and medication type being exposed.

Outcome:

Packers were retrained in proper packing procedures.
