Based on interview and record review, the facility failed to maintain the privacy and confidentiality for one patient (Patient A), when an employee mistakenly sent Patient A ' s insurance information to the wrong medical group.Findings:An interview was conducted on 3/21/12 at 2:00 PM, with the facility ' s Privacy Officer (PO). The PO stated that on 1/25/12, she was notified that an employee had sent Patient A ' s insurance information to the wrong medical group.The PO further stated that the breach was identified when the medical group called the facility and let them know that Patient A was not a member.A review on 3/21/12 of the personal health information (PHI) for Patient A that was disclosed in error included; Patient name Date of Birth Age Address Employer Physician nameThe PO further stated on 3/21/12, that Patient A was notified of the breach. The PO stated that the employee failed to verify the correct information prior to sending it.
