Issue:

The mailing label on supplies that were sent to the Veteran contained the Veteran's SSN. The Veteran and his spouse is very upset that this occurred. The Veteran & his wife checked with their bank about a possible breach, however at this time there was no problems with the bank account reported. The information about this event was reported by the Prosthetics and Pharmacy service. Based on a system that was used approx 7 years ago it was found that the mailing labels contained SSNs. This was corrected, however the change for this Veteran did not take within the system. This has now been corrected. Update: 01/17/13:Veteran A will be sent a letter offering credit protection services.

Outcome:

Mailing label program corrected. Pharmacy techincian checks labels to ensure that SSNs are not printed on mailing label.
