Issue:
The Schedule of Operations List, parking stickers, and parking sticker validation lists are missing. Information is routinely secured in a locked drawer at end of each shift. Only two individuals have keys to the drawer. Information was not in the drawer when the other clerk assumed duties. Update: 04/05/12:The list contained 9 Patients with full name, full SSN, and medical information. The parking sticker list had patients last name and last 4 of SSN. Therefore 9 Patients will receive a letter offering credit protection services.
Outcome:
We have recovered the information. We have also re-emphasized to the staff the importance of protecting our patients information. All hard copy patient information has been removed from that area and is no longer stored there.
