Issue:
A Veteran stated that "I know people are going into my records and I want a copy." The Veteran also called names of employees and requested to know everyone who has access to his records. He requested and was given Sensitive Patient Access Report (SPAR) on 03/07/12 by the Privacy Officer (PO) who received signed SPAR from the Veteran on 03/08/12. Update: 05/18/12:The Information Security Officer reviewed the access to the Veterans record. Each access was justifiable and related to the Veterans care. No breach has occurred.11/08/12:It was confirmed that 3 employees impermissibly accessed the complainant's health record and received disciplinary action. Please re-evaluate this event for action.11/14/12:The Veteran will receive a letter offering credit protection services.

Three employees received disciplinary action.
