Issue:

On, 02/17/15, the Privacy Officer (PO) was notified that a deceased Veterans spouse received records for two other Veterans in September 2014. A review of the Release of Information (ROI) software confirms 876 pages of old medical records from the Federal Records Center (FRC) and 3 additional pages of old charts were provided in this request. The Veterans wife did not report record receipt of other records to facility, however, wife reported and provided records to Senator McCain, who contacted the facility. Additional notification and investigation, to ensue.

Outcome:

02/24/15: The Incident Resolution Service Team has determined that one Veteran will be sent a letter offering credit protection services, and that the other Veteran's Next of Kin will be sent a notification letter.
