Issue:
A VA contract provider handed a Veteran his documentation and it had documentation on 3 other Veterans attached. The Veteran realized it was not his information, brought it back to the clinic and reported it. The documentation contained the 3 Veterans' names, addresses, and full SSNs. Update: 03/14/13:Three Veterans will be sent a letter offering credit protection services.
