Issue:

Based on interview and record review, the hospital failed to safeguard Patient A's protected health information. Findings:On 3/7/11, the hospital reported to the Department an accidental disclosure of Patient A's medical record occured.On 8/3/11 at 0920 hours, the hospital's Compliance & Privacy Officer was interviewed. The Officer stated Patient A was an organ donor. The hospital's investigation showed a laboratory report for the patient was sent to the wrong physician. Hospital staff failed to verify the physician's address prior to mailing the laboratory test result. The breach of information was discovered on 2/24/11.
