Issue:

This incident occurred at a Contracted CBOC in Columbia, TN (Maury County). The Privacy Official for Humana notified the VHA BAA Office of this incident who then forwared to the TVHS Privacy Officer to log. On April 30th, a Contract Employee, while completing a prescription for medication for Veteran A, inadvertantly had Veteran B's information on the computer and completed the prescription with Veteran B's information and then provided the prescription to Veteran A. The clinic realized the error and contacted Veteran A, who immediately returned the prescription back to the clinic. Information of Veteran A which was on the written prescription included Full name, Date of Birth, Address and the medication information.

Outcome:

05/06/14: The Incident Resolution Team has determined that Veteran A will be sent a letter offering credit protection services due to name and DoB being disclosed.
