Issue:

Veteran A received the chest x-ray results of Veteran B along with her lab results. Both results were sent in the same envelope to Veteran A. Veteran A contacted the facility Privacy Officer to report this and Veteran A was instructed to return the paper to the facility. They were returned on March 15, 2012. Individually Identifiable Information included full name and mailing address only and did not include the date of birth or SSN. Update: 03/19/12:Veteran B will be sent a HIPAA notification letter.
