Issue:

Veteran A accidentally picked up and took home a copy of patient schedule (with full names, SSNs) and printed prescription for Veteran B (with full name, SSN). It appears the Veteran placed copies of his lab results on the providers workstation desk and when he picked his lab results up to leave, he inadvertently picked up the patient schedule and prescription. The Veteran contacted the provider and agreed to place this sensitive information in the mail addressed back to the provider. The Veteran is unable to bring the documents back to the CBOC in person as he lives some distance away and does not have transportation. Once the CBOC manager receives the documents in question, he will immediately notify and forward the information to the Privacy Officer. Still under investigation upon receipt of information Update: 03/16/12:Once Veteran will be sent a letter offering credit protection services.04/12/12:The Privacy Officer received the papers from the Veteran and totaled 21 Veterans on the appointment list.

Outcome:

Credit Monitoring Letter signed by Director, put in mail 4/17/12 and should go out this same afternoon. Time delay due to patient A living 2 hrs from the CBOC and waiting for patient A to mail back the papers accidentally picked up.
