Issue:

The Information Security Officer reported that a call was received from a City Police Officer that 16 patients records were found in a house of an former employee who is now deceased. Update: 05/11/12: The deceased employee was a nurse and it is not known how the records were in her house. They have been recovered by the Privacy Officer and are back at the VA. Letters offering credit protection services will be sent to 16 Veterans.

Outcome:

16 medical records were found in the house (in the attic) of a former Dayton VAMC Employee, who is now deceased. The records were discovered by a non-VA employee (new house owner) and reported to the local police department, according to police report. Records were picked up from the police department by Medical Center staff. After a thorough investigation and according to the police report, it cannot be determine how and why the records were removed from the Medical center. 16 notification letters signed and mailed out to Veterans June 25, 2012. Ticket re-open status changed form pending notification to pending credit monitoring 6 deaths including. 10 creditoring monitoring letters were reviewed, signed and mailed on July 6, 2012. Six deaths: Two confirmed NOK (death) letters mailed - July 6, 2012 One both Veteran and NOK (wife) - deceased, unable to locate NOK. Three NOK are listed as friends - no letters mailed - unable to locate NOK. Unable to determine how and why the records were removed from the Medical Center.
