Issue:

Based on interview and record review, the facility failed to safeguard Patient 1's Protected Health Information (PHI) from unauthorized persons which resulted in a breach of her PHI.Findings:During an interview with the Privacy Officer (PO) on 6/20/13 at 9:30 AM, she stated Staff A faxed Patient 1's PHI to a private company when she used the wrong fax number. The PO stated Staff A should have verified the fax number with another staff member before faxing the PHI which was the facility's policy. The information provided by the PO indicated the following PHI for Patient 1 was breached: Name, address, date of birth, telephone number, insurance information, diagnosis, charges, X-ray report, age, and sex.
