Issue:
Veteran A was given Veteran B's discharge instructions upon discharge from Urgent Care. The documents contained Veteran B's name, full SSN and discharge instructions. Update: 06/04/12:Veteran B will be sent a letter offering credit protection services.NOTE: There were a total of 112 Mis-Handling incidents this reporting period. Because of repetition, the other 111 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
The responsible employee was required to re-take the appropriate training modules. The employee's Supervisor was instructed to consult with Human Resources (HR) to determine if any disciplinary action is deemed necessary.
