Issue:

Veteran A reported that he inappropriately received copies of Veteran B's medical record. Veteran A reported and relinquished to a CBOC clerk an envelope containing one Progress Note belonging to Veteran B consisting of 4 pages; one VAF 10-5345a, signed authorization of Veteran B consisting of 1 page; cover letter addressed to Veteran B; and one CD containing dental images of Veteran B.

Outcome:

06/17/13: Veteran B will be sent a letter offering credit protection services. NOTE: There were a total of 24 Mis-Handling incidents this reporting period. Because of repetition, the other 23 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
