VA Employee found a copy of the Inpatient Roster listing the full name, last 4,digits of the SSN age, date of admission, ward, and room-bed number for 4 patients. The list was found on the ground on the 3rd floor atrium. Update: 08/24/12:The list was in a public area for 24 hours or less. Four patients will be sent a letter of notification.
Outcome:
Supervisor has been notified and appropriate action taken. Copies of letters scanned.
