Issue:

Today, while at an appointment at the Womens Clinic, Charlotte Ave, Veteran A was placed in an exam room to await the provider. While in the exam room, Veteran A observed documents pertaining to two other Veterans left unattended on the desk. Veteran A stated she was left alone in the room with the documents for approximately 45 minutes. Veteran A took a photo of the documents with her cell phone which she showed to the Privacy Officer (PO), Nashville Campus. Only the top page of the documents was visible to the PO, Nashville Campus. This document appeared to be a clinical reminder sheet and contained the full name, full social security number, and date of birth on two Veterans. Veteran A stated she will delete the photo when she is satisfied appropriate action has been taken regarding the employees who are responsible for leaving the documents unattended

Outcome:

12/03/14: The Incident Resolution Service Team has determined that two Veterans will be sent letters offering credit protection services.
