Issue:

Based on staff interview, clinical record and administrative document review, the hospital failed to keep Protected Health Information (PHI) confidential when Patient 1's mammography results were mailed to the incorrect patient. This failure caused the breach of Patients 1's PHI and the potential for unauthorized use.Findings:On 1/9/15 at 11:31 a.m., during an interview, the Privacy Officer (PO) stated the hospital's computer generated notification system printed Patient 1's mammography results, along with results intended for another patient. The PO stated this was a "glitch" in the computer system, that the hospital had been previously unaware of, which has now been corrected.The PHI breached included Patient 1's name, address, phone number, medical record number, and clinical findings.The facility policy and procedure titled "Confidentiality of Protected Health Information" dated 8/11/14, contained the following documentation: "Policy Summary/Intent: ... is committed to protecting the privacy and security of protected health information (PHI). Policy: Compliance-Key Elements: It is the policy of ... to maintain confidentiality for patients and employees at all times and under all circumstances."
