Issue:
Veteran A brought documents to the person at the check in desk that had been given to him at an appointment several days earlier. He was upset because there was a diagnosis on the routing slip that he had never had before and he was prescribed medication for that diagnosis. The preliminary investigation appears to indicate that the provider handed Veteran A his routing slip and also that of Veteran B who had been seen near the same time. The Privacy Officer (PO) is still waiting for a couple of other statements from employees involved to make the final determination Update: 04/18/12:Veteran B will be sent a letter offering credit protection services due to full name and full SSN being disclosed.
Outcome:
Re education has been provided to the involved staff. We were unable to determine when the actually breach occurred, but we believe that the information was given during the visit with the provider. The processes for check-out have been reviewed and will be more closely monitored.
