Issue:

A letter was sent out from Home Based Primary Care (HBPC) to Veteran A. Along with his letter, Veteran A received Veteran B's medication list. Veteran A's daughter notifed the VA. The Privacy Officer has left messages for Veteran B to try and determine if he received the wrong medication list but has not been able to reach him at this time. There could possibly be two violations. Update: 08/22/11:It was verified that only 1 patient received the wrong information, therefore Veteran B will receive a letter offering credit protection service due to full SSN being disclosed along with medication information.

Outcome:

Unable to determine who made the error. Staff will be provided education and training to ensure correct information is mailed to correct Veteran.
