Issue:
Based on interview and record review, the facility failed to ensure a label containing protected health information (PHI) for Patient A, was not placed on admission papers for Patient B. This failure resulted in the unauthorized disclosure of Patient A's PHI, which created the potential for unauthorized use.Findings:On April 30, 2012, at 11 a.m., a telephone call was placed to the facility Compliance Specialist. The Specialist stated the label for Patient A was placed on the document, "Conditions of Admission," for Patient B, on April 20, 2012. The label contained Patient A's name, date of birth, age, medical record number, and account number.On April 30, 2012, the facility policy and procedure titled, "Patient Identification Policy ad Procedure," was reviewed. The policy indicated, "Purpose: To accurately identify the individual as the person for whom the service or treatment is intended and to match the service or treatment to that individual..."
