Issue:

A VA Housekeeper found a Motor Vehicle Operator exam certificate on a VA volunteer in a waiting area with a main hallway open to the public. The information contained full name, full social security number, date of birth and information about driving qualifications. VA employee turned the information over to the VA Police to file a report and gave the certificate to the Privacy Officer (PO). Update: 02/28/13:The volunteer will receive a letter offering credit protection services.
