Issue:
On, 6/11/2015, Veteran reports a privacy complaint to PO. Veteran had previously contacted both Patient Advocate and PO who had returned messages, making contact today. Veteran describes that her address had been erroneously changed, resulting in misdirected mail to another state by Tri-West. Mail was returned to Tri-West who sent it to her sister. Upon letter receipt, Veteran contacted facility for address correction. Veteran requests investigation into whom and why address was changed with information regarding possible extent of sensitive information that was sent to incorrect address. Notice of Privacy Complaint and Acknowledgement letter, en- route. Additional notification and investigation, to ensue.
Outcome:
Based on the Veteran's documentation provided showing that we sent a letter to this erroneous address and the supervisor's investigation confirming that we changed an accurate address to this address, ticket was changed to an Incident. 06/23/15: The Incident Resolution Service Team has determined that Veteran A will be sent a general notification letter.
