Issue:

Veteran A came to the pharmacy window to pick up his medication it was not ready, he was ask to step to the side and the tech waited on Veteran B. The tech motioned to Veteran A to come to the window that his medication was ready. He pickup the medications of Veteran B, who hadn't taken his medications out of the box yet. Then Veteran A left the facility before the pharmacy could stop him. They contacted Veteran A at his work site and he returned the medication. Update: 12/28/11:Veteran B will be sent a notification letter.

Outcome:

Supervisor counseled pharmacy technician to be more attentive to what each Veteran was picking up at the window.
