Issue:
Veteran Spouse brought an appointment letter back to the registration window which had been given to her husband with the wrong patient name and last four SSN. The appointment was entered on wrong Veteran. The letter had not left the VA, as the Veteran had just left the clinic when his spouse realized the name on the letter was wrong. Update: 07/01/11:Veteran B will be sent a notification letter.
Outcome:
Employee responsible reminded of the need for caution when handling documents with PII & confirming patient ID.
