Issue:
During an appointment with a Veteran, the provider laid the appointment list for her clinic on the desk. When the Veteran left, she noticed the list was gone. She attempted to locate the Veteran before he left but failed to catch up with him. She did call the Veteran and he reported that he noticed the list on his way home and destroyed (burned) it when he got home. The provider reproduced the list and faxed it to the Privacy Officer. The list contains the Veterans' names, full SSN and diagnosis. Update: 10/03/11:Nine (9) Patients will receive letters offering credit protection services due to full name and SSN being exposed.
