Issue:
Employee A reported 2 privacy incidents. First, the Human Resources (HR) Department mailed her a printout of her individual retirement record. When Employee A received the envelope at her house, the envelope was not sealed, but there does not appear to be any tampering with the envelope. Employee A is concerned that someone may have her information from the opened envelope (full name, full DOB, address, full SSN, and her retirement savings amounts) because the HR Department had stated that they had mailed the envelope on 02/25/13 but the envelope was not postmarked until 03/07/13. Second, attached to Employee A's retirement record, was a sheet from Employee B's retirement record, which contained Employee B's full name, full DOB, full SSN, and the dates of military service. Update: 03/14/13:Both employees will receive a letter offering credit protection services.
Outcome:
Staff education done regarding mailing procedures. Credit Monitoring letters sent to Employee A and to Employee B.
