Issue:
Based on interview and record review, the facility failed to prevent unauthorized access to a patient's (Patient A) PHI (Protected Health Information).Findings: On March 4, 2013, a visitor requested a copy of the cafeteria's menu from the facility volunteer. When the visitor returned to the patient's room, it was realized the visitor has the day's census for 45 patients. The census included, patients' names, dates of birth, age, gender, date of admission, room number, bed number, and religion code.In an interview with the facility's CO (compliance officer) she stated, "The volunteer at the information desk was asked for a menu by a visitor. A hospital census was given also." The visitor and the patient notified the nurse.
