Issue:
A Release of Information (ROI) clerk released the wrong record to a requestor. This was discovered when Veteran A asked for an accounting of disclosures. Veteran B's name, address, full SSN and protected health information (PHI) were disclosed. Update: 03/02/12:Veteran B will receive a letter offering credit protection services.NOTE: There were a total of 25 Mis-Handling incidents this reporting period. Because of repetition, the other 24 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Reviewed with ROI staff on importance of details. Privacy training redone.
